Subject: EMPLOYEE RIGHT TO REVIEW PERSONNEL FILE

1. Employees may review the following material in their personnel file by scheduling an appointment with the Human Resources Department. The review of a file should be in the presence of a Human Resources administrator to ensure that any questions which may surface during the review are promptly and accurately answered.

a. Application/resume;
b. Grade transcripts;
c. All correspondence subsequent to date of employment;
d. All evaluations conducted during employment with MMSD.

2. Employee may not review the following:

a. Reference letters received in conjunction with the application for employment;|
b. Reference information obtained by the Human Resources Department (e.g., Rating Scale for teachers);
c. Credential ratings;
d. Pre-employment interview evaluations.

Requests for Copies of Materials

Human Resources will provide one (1) copy of materials requested by employees, excluding the pre-employment materials excluded from review above.

Requests for copies shall be honored as soon as practicable and without delay and in accordance with an established fee schedule relative to the reproduction cost for public records. A list of such fees shall be made available at the Office of the Director of Human Resources. Should the fee be in excess of $5.00, it shall be paid in advance of materials being copies.
