ADMINISTRATORS, NON-REPRESENTED EMPLOYEES, TRADES AND AFSCME-REPRESENTED EMPLOYEES (COMBINED GROUP)

A. Choice of Plans

The Combined Insurance Group (Administrators, Non-Represented Employees, Trades and AFSCME represented employees) may choose health insurance from the plans offered at the time of employment or during the open enrollment period and may change insurance coverage during the annual choice period.

B. Eligibility

All regular and permanent bi-weekly employees working 19 hours or more per week or administrators with a 50% or more contract are eligible for coverage under one of the District's health plan options available to the Combined Group. Coverage is effective the first day of the month following one month of employment, without an additional waiting period, if they complete and return the health insurance form to the Department of Human Resources within one month of their first day of employment.

C. Late Enrollment

Late enrollment into dental insurance is effective on the first of the month following a three-month wait. The waiting period begins on the date the dental application is received by the Benefits Division.

EMPLOYEES REPRESENTED BY MADISON TEACHERS, INCORPORATED

A. Choice of Plans

All employees represented by Madison Teachers Incorporated may choose health insurance from the plans offered at the time of employment or during the open enrollment period and may change insurance coverage during the annual choice period.

B. Eligibility

All teachers who have a 50% or more contract and all other regular and permanent bi-weekly employees represented by MTI who work 19 hours or more per week are eligible for coverage under one of the health plans available to MTI represented employees.

Coverage is effective the first day of the month following one month of employment, without additional waiting period, if the employee completes and returns the health insurance application form to the Department of Human Resources within one month of his/her first day of employment.

C. Late Enrollment

Late enrollment into dental insurance is effective on the first of the month following a three-month wait. The waiting period begins on the date the dental application is received by the Benefits Division.
