Obtaining a Work Permit

• Your birth certificate
• A letter from the employer saying he/she intends to hire you and listing job duties, hours of work, and time of day you will work.
• A letter from the parent/guardian giving permission for you to accept the job (this may be a parent note on the employer’s letter)-Your Social Security card and
• $5.00 in cash or a check for $5.00 made out to La Follette High School
