REQUESTING A TRANSCRIPT

Current Shabazz City High School students must complete a Transcript Request Card available in the Guidance Office or Main Office.

A parent signature is required for a student under age 18.  There is no charge for the first two transcripts.  Thereafter, each transcript charge is $1.00.  (Make check payable to Shabazz City High School)

Graduates may come to the Main Office or Guidance Office and complete a Transcript Request Card, or they can mail or fax a signed request to the school.  Phone requests cannot be accepted.

Please note - If you need a transcript during the months of June, July and August you will need to mail a signed release form to your home high school as Shabazz Office is closed during these months:
