Update your Address NOW for your 2005 W-2 Form

W-2 forms will be delivered in January 2006. If your payroll check is sent to your school, so will your W-2. If your payroll check is sent to your home, so will your W-2. It is your responsibility to make sure the District has the correct address for you. Reminder: There is a $10 fee to have a duplicate W-2 produced.

• January 15 - Deadline for Administrator's Retirement Notification
• February 1 - Deadline for Sabbatical leave requests for 1^st semester 06-07
• February 15 - Deadline for Teacher Retirement Notification
• April 15 - Non-Medical leave request deadline for 2006-2007 (MTI teacher CBA)
• April 23 - Final day to use Paid Personal Leave day (MTI teacher CBA)
• April 23 - Final day to use School Visitation day (MTI teacher CBA)

Retirement deadlines
(retirement letter and forms received in human resources):
Administrators by January 15, 2005
Teachers by February 15, 2005
All others, no later than two weeks prior to retirement
