Predetermination of Benefits

Though it has never been a requirement in our Dental plans, Delta Dental offers a service to you and your dentist called Predetermination of Benefits. After an examination, your dentist may recommend a treatment plan. If the proposed treatment plan involves crowns, onlays, fixed bridgework, or orthodontics, ask your dentist to send the treatment plan with x-rays to Delta Dental. Your treatment plan will be reviewed by Delta's professional review team, and your available contractual coverage will be calculated and printed on a Predetermination of Benefits form. Copies of the form will be sent to you and to your dentist.

The Predetermination of Benefits service from Delta Dental will help you make informed choices about your dental care. And, you'll be aware of your out of pocket costs relating to the proposed treatment.
