HEALTH SERVICES
Nurse - Conny Mares
Nurse Assistant
Phone: 204-4759
The school health office for the middle school is open daily. If a student
feels ill or gets hurt during the school day, (s)he is required to have
a pass from a teacher to go to the health office. The nurse or nurse’s
assistant will collect information and do an assessment to determine what
actions need to be taken. The health office personnel will be responsible
for contacting the parent, if necessary. No student is allowed to carry
medications without consent of the health office. Tylenol/Ibuprofen is
available in the health office for students with a signed, written permission
form. Students may also check with school nurse during breaks and before
or after school to schedule appointments for health concerns. Students
should report to the main office if staff is not available in the health
office. The school nurse also helps to assure that children who have acute
and chronic health care needs are able to learn. The school nurse works
with staff to meet the health needs of students while on fieldtrips. The
nurse is involved in health education, student assessment in both the
health office and classroom, and in community outreach programs.
School nurses in all middle and high schools are able to dispense acetaminophen
(Tylenol) or ibuprofen (Advil or Motrin IB) to students with headaches
, other discomforts, or menstrual cramps. Your child will only be able
to receive these medications with written parent/guardian permission and
subject to the availability of the school nurse. The dosage of medication
is specified in “standing orders” approved by the school district
medical consultant, Greg DeMuri, M.D.
1. For HEADACHE OR DISCOMFORT, school nurses may give: Acetaminophen based
on weight guideline of approximately 15 mg/kg/dose every 4-6 hours as
needed with maximum single dose of 1000 mg. OR Ibuprofen based on weight
guideline of 10 mg/kg/dose every 6 hours as needed with maximum single
dose of 400 mg.
2. For DYSMENORRHEA, school nurses may give: Ibuprofen based on weight
guidelines 10 mg/kg/dose every 6 hours as needed withmaximum single dose
of 600 mg. In general, for students weighing less than 88 pounds, use
400 mg per dose.
Forms can be requested at registration or from the Nurse’s office.
Other medications used or administered in the school setting require a
physician’s order form and parent consent form. These need to be
on file with the school nurse before the medication can be administered.
New forms are required each school year and can be obtained from the school
nurse.

Student Services
The student services group consists of the psychologist, social worker,

ways of meeting the needs of individual students. These services may include
consultation regarding social, emotional and behavioral issues that may
be interfering with learning. Staff participate in IEP Team evaluations
and building level assessment.
Students and parents are welcome to consult with these staff. They can
provide students with help regarding personal and interpersonal problems,
academic problems, school program issues, and specialized testing. Student
